Healthier Moms and Babies Launching Nurse Family Partnership

Thanks to a grant from the Indiana State Department of Health, Healthier Moms and Babies has been awarded funding to implement Nurse-Family Partnership in Allen County.  Nurse-Family Partnership helps a woman in poverty pregnant with her first child become the best mom she can be. Each expectant mom connects with her own personal nurse who works alongside her throughout her pregnancy until her child’s second birthday to help her accomplish her goals and give her child a healthier start. Each Nurse-Family Partnership nurse guides the expectant mom to improve her prenatal care – helping the new mom to have a healthy pregnancy, improve birth outcomes and her child’s health and development. The program is free and voluntary for women that qualify.

The nurse also supports the new mom to become economically self-sufficient – encouraging her to continue school and pursue her career goals. By strengthening families, Nurse-Family Partnership sparks multi generational change and helps break cycles of poverty.

Over the next several months, Healthier Moms and Babies will be hiring our nurse case management team. Currently we are hiring for the Nurse Family Partnership Supervisor.  Click to learn more.

Nurse Family Partnership will be another program Healthier Moms and Babies offers in addition to our core home visitation program for either medical or social at-risk expectant mothers, Baby and Me Tobacco Free and Cribs for Kids. We have a centralized intake which will determine eligibility for our programs.
