Healthier Moms and Babies is a nonprofit organization whose mission is to reduce infant mortality and improve outcomes in our community. Healthier Moms and Babies provides several comprehensive programs which are designed to educate, empower, and inspire participants to make positive changes to impact the outcome of the pregnancies, their lives and the lives of their families.


Job Purpose

Within Healthier Moms and Babies (HMB) Healthy Start and reporting to the Program Supervisor, the Healthy Start Family Advocate provides health education and case management services to low-income, high-risk pregnant women in Allen County, visiting them in their home to improve the outcome of pregnancies and reduce infant mortality.

    • Conduct medical-social needs screens and assessments through a one-to-one interview process with all appropriate referrals. May meet with clients in a variety of locations including but not limited to the following: private homes, hospitals, clinics, and community agencies.

    • Provide documentation of home visits and screens accurately and timely.

    • Meet with supervisor weekly to review cases.

    • Serve as an advocate for participants and potential participants by scheduling prenatal appointments, arranging for Medicaid taxi, and referring to appropriate community resources.

    • Provide health education and case management services; assess the needs of pregnant women and their families; develop a comprehensive care plan with the woman to meet her needs and improve the outcome of her pregnancy and provide the woman and her family access to social and health services, advocate for her when necessary.

    • Conduct visits with families on a weekly, bi-weekly, or monthly basis or as mandated by HMB policy.

    • Maintain tracking forms, required statistical information and other paperwork as required, in a detailed timely manner.

    • Conduct prenatal health education sessions for community resources.

    • With the supervisor, collaborates with community partners to maintain an effective referral base.

    • Assist the Assessment Worker with conducting eligibility home visit as needed.

    • Assist with fundraising and community activities, as needed.

    • Respond to client emergencies, as needed and appropriate.

    • Demonstrate or can quickly learn the social service delivery system  

    • Communicate clearly, concisely, and in a tactful, professional manner.

    • Share accurate information about Healthier Moms and Babies.

    • Take initiative and responsibility for decisions as an individual and as a staff member of HMB. Encourage a positive work environment and experience for co-workers and clients.

    • Adhere to all organizational and departmental policies, procedures, and practices; maintain proper attendance and punctuality.

    • Work evenings and weekends as required.

    • Utilize material, equipment, and time in a safe, beneficial, and cost-effective manner; organize workload to complete responsibilities in an appropriate and timely manner.

    • Participate in in-service training and attend conferences as needed.

    • Adhere to ethical business practices by striving to perform in a manner that conforms to the highest standards of ethical behavior, integrity, and honesty; exhibit commitment and promote HMB’s mission, values, and vision.

    • Adhere to all organizational and departmental policies, procedures, and practices; maintain proper attendance and punctuality.

    • Perform other duties as assigned by Supervisor.

    • BA/BS Degree

    • Minimum of a Bachelor’s degree in Human Behavioral Science, Social Work or Human Services. Education focus or education minor on drug addiction, alcohol addiction, education, counseling, psychology, child development, family intervention, therapeutic techniques, preferred.

    • Must have a current valid Driver’s License.

    • Minimum of two years experience required

    • Case Management, preferably with home visits, experience with maternal child health; familiarity with local social service delivery network preferred. Previous experience in case management, home health, maternal child health or other work directly related to the mission of HMB preferred.

    • Knowledge of pregnancy, labor and delivery, infancy, of community agencies and social service network, ability to work with diverse cultures and a willingness to make home visits.

    • Demonstrates integrity in all situations and interactions; has the ability to develop trusting, collaborative, professional relationships with coworkers, clients, and community collaborators.

    • Relates in a culturally sensitive manner to low-income clients and those from diverse backgrounds.

    • Demonstrates a passion for excellence and a commitment to lifelong learning.

    • Able to communicate clearly, concisely, and in a tactful, professional manner.

    • Demonstrates initiative, self-motivation, and pro-active behavior.

    • Knowledge of community agencies and social service network, ability to  work with diverse cultures and a willingness to make home visits. Must have superior conflict resolution and/or mediation skills.

    • Must have organizational skills; demonstrated ability to manage multiple and diverse clients concurrently.

    • Must have strong analytical and problem-solving skills.

    • Computer literacy in Microsoft Outlook, Office, and Excel required.
