Healthier Moms and Babies provides several comprehensive programs which are designed to educate, empower, and inspire participants to make positive changes to impact the outcome of the pregnancies, their lives and the lives of their families.
Job Purpose
Within Healthier Moms and Babies (HMB) and reporting to the Intake & Outreach Supervisor, the Intake & Referral Worker is responsible for reviewing all referrals of prospective participants to determine eligibility for programs offered by HMB. Meets with participants to complete enrollment documentation and provide brief services as necessary. Provides prenatal care coordination and referrals for extra supportive services if needed. The Intake & Referral Worker will also provide health education and case management services to low-income, high-risk pregnant women. This role will assist and participate in HMB’s diaper distribution program and activities to engage clients.

Conducts medical-social needs screens and assessments through a one-to-one interview process with all appropriate referrals. May meet with clients in a variety of locations including but not limited to the following: Private homes, hospitals, clinics, and community agencies.
Provide documentation of assessment and screens accurately and timely.
Meets with supervisor weekly to review assessment information and appropriateness of referral services to HMB.
Plans and provides short-term brief services until assigned to an HMB ongoing Home Visitor.
As needed will provide ongoing case management by conducting home visits with families on a weekly, bi-weekly or monthly basis or as mandated by HMB policy as needed.
Provide health education, case management or brief services; assess the needs of pregnant women and their families; develop a comprehensive care plan with client to meet her needs and improve the outcome of her pregnancy and provide client and her family access to social and health services, advocates for client when necessary.
Maintains tracking forms, required statistical information and other paperwork as required, in a detailed and timely manner.
Accurately and in a timely fashion processes new client referrals and enters data into the electronic database.
Prioritizes work according to referral requirements and deadlines. Works to exceed established productivity standards/goals.
Assist with diaper distribution activities which may include helping with fulfillment of diaper requests, client diaper distribution, and attending Hopper outreach activities.
Recruit perspective clients for HMB services including group education classes.
Assists with community activities, as needed.
Help facilitate group education classes as needed.
Respond to client emergencies, as needed and appropriate.
Demonstrates or can quickly learn the social service delivery system for HMB service area.
Communicates clearly, concisely, and in a tactful, professional manner.
Shares accurate information about Healthier Moms and Babies.
Takes initiative and responsibility for decisions as an individual and as a staff member of HMB. Encourage a positive work environment and experience for co- workers and clients.
Adhere to all organizational and departmental policies, procedures, and practices; maintain proper attendance and punctuality.
Utilize material, equipment, and time in a safe, beneficial and cost-effective manner; organize workload to complete responsibilities in an appropriate and timely manner.
Participate in in-service training and attend conferences as needed.
Adhere to ethical business practices by striving to perform in a manner that conforms to the highest standards of ethical behavior, integrity and honesty; exhibits commitment and promotes HMB’s mission, values and vision.
Work evenings and weekends as required.
Report any case that may involve “imminent harm” or “threatened harm” to a supervisor immediately. Always intervene on behalf of the child when necessary.
Perform other duties as assigned by Supervisor.

BA/BS Degree
Minimum of a Bachelor’s degree in Social Services or related field required.

Valid driver’s license, auto insurance and vehicle for full time business use required.

Minimum of two years’ related experience required
Previous experience with intake/assessment and/or case management, home visiting, maternal child health, familiarity with local social service delivery network or other work directly related to the mission of HMB preferred.
Requires flexibility to work afternoon or evenings as dictated by program needs.

Computer literacy in Microsoft Outlook, Office, and Excel required
